Using structured evaluation forms ensures everyone evaluates candidates using the same criteria. This makes it easier to compare and summarize feedback and make better hiring decisions.
Once you’ve created an evaluation form, you can immediately begin using it to collect feedback.
Add them to your evaluation kit:
When creating a new job or editing an existing one, go to your job settings sidebar and click Evaluation kit to link forms to specific pipeline stages. This helps your team use the right evaluation forms at the right stage of the hiring process.
Evaluate or request directly from a profile:
Go to a candidate’s profile and open the Evaluation tab. From here, you can either complete an evaluation form yourself or assign it to other team members.
Create an automation:
Set up an automation to send evaluation requests to the right team members as candidates move through the hiring process.
Connect forms to the Event scheduler:
When scheduling candidate interviews with the Event scheduler, you can request that team members complete an evaluation form after the interview.
Note: Evaluation forms are available on the Advance and Optimize plans. If you’re on the Start plan, you can use Quick Evaluations instead.
Create a new evaluation form
Build a structured form template from scratch that aligns with your hiring process.
1. Go to Settings > Templates > Evaluation forms.
2. Click New template.
3. Enter a name for your template and select an optional category. Click Create.
4. Use the rich-text editor at the top of the form creator to provide context for your hiring team. You can use this space to outline the evaluation’s purpose or to share specific guidelines.
5. Click + Add new to start building your structured form.
Note: Every evaluation form has a default Evaluation score at the bottom. The score here will determine the candidate’s overall rating.
To customize your evaluation scale, click the gear icon. You can choose between a thumbs-based rating or a numbered scale. You can also decide whether this final question is required or optional.
Form elements
You can add various elements from the dropdown menu to customize your evaluation form:
Add questions
Text (single line): An open-ended question that requires a short answer.
Text (multiple lines): An open-ended question that allows for a longer answer.
Yes / No: A simple binary question for quick confirmation (e.g., “Is the candidate willing to relocate?”).
Single choice: A question with multiple choices where the evaluator can select only one answer.
Multiple choice: A question with multiple choices where the evaluator can select one or more answers.
Drop-down: A compact, selectable list where the evaluator chooses a single answer from a drop-down menu.
Date: A field that prompts the evaluator to input a specific calendar date (e.g., target start date).
Salary: A field to input salary expectations or other compensation figures discussed during the interview.
Add a file: Allows the evaluator to upload a document directly to the evaluation (e.g., a portfolio sample or test assignment).
Tip: You can mark a question as required with the asterisk icon, create a copy with the duplicate icon, delete it with the trash icon, or move it up or down with the arrow icons.
Add ratings
Rating scale: Allow reviewers to select a rating from five visual options (numeric, thumbs, hearts, stars, or emojis).
Skill evaluation (Tellent HR): If your company uses Tellent HR (Grow), you can add a skill evaluation section to your evaluation forms. The skills linked to the position are automatically displayed when an interviewer completes the evaluation.
Scorecards: A list used to rate multiple criteria under a single topic. You can add a main heading or question, include a helpful hint, and list separate items with optional descriptions to guide your team members.
Add structure
Section: A visual divider you can use to break your form into organized parts, such as different categories, topics, or phases of an interview.
Info box: Use this to add notes, instructions, reference links, or other helpful information for interviewers.
Edit question visibility
By default, everyone with access to an evaluation can view all questions and answers. If a question contains sensitive information, you can limit who can see the responses.
Note: Answers to private questions are automatically excluded from AI-generated summaries.
Note: The Evaluation score and Skill evaluation (Tellent HR) rating can’t be made private and are always visible.
To make a question private:
1. At the bottom of the question, click the Visible for everyone dropdown.
2. Click Selected members.
3. Choose the people or roles who should be able to view the answers.
Only the selected people and roles will be able to see responses to that question.
